How do I print my parent’s Income Verification form?

Please print a copy of the income verification form if you are under the age of 24 AND you are:

• Not married or
• Not supporting a child or
• Not supporting other dependents (excluding spouse or child)

Before coming to your appointment at the Lee College Educational Opportunity Center, you must have A PARENT COMPLETE AND SIGN the Income Verification form. Be sure to bring the signed and completed form with you to your appointment. If you are over the age of 24, none of this applies to you. If you have questions, please contact the Lee College Educational Opportunity Center at 281-421-0553. This information is required as part of the regulations that govern the EOC grant.
