The O.I.T. forms link provides authorizations forms
for New Accounts (Internet, Email, Peoplesoft
Financials and PeopleSoft Student Records Database). If Lee College
property (LC tag) is removed for any reason, the Property Removal forms
must be completed and approved prior to LC property being removed. Laboratory Operational
Readiness Testing (L.O.R.T.) forms are for the
technicians to certify the functionality of a computing lab. This
form is approved by the Division Chair as well as the Lead Instructor
for that lab. Telephone Request forms are use
to request a new analog/digital telephone line or a Long Distance Access
