Course Reserves

Reserve Request Form

Please fill out a form for each reserve item and allow one week for reserves to be processed and available to your students. If you have multiple items to place on reserve, please allow additional time. Materials will be processed in the order in which they are received.

BOOKS: author, title, publisher, and year of publication.

BOOK CHAPTERS: author, title of book, chapter author (if noted), title of chapter, page numbers, publisher, year of publication, AND copy of the copyright information page in the book.

JOURNAL ARTICLES: journal title, issue date, article author, article title, page numbers, AND copy of copyright information page.
