Required Points

• Apply for admission to Lee College, submit to the Admission and Records office an official transcript from high school or GED certificate, and any/all colleges attended,
• Application for VN program on file in Allied Health office,
• Copy of high school diploma or GED certificate in VN folder in Allied Health office,
• Degree plan from Counseling Center signed by counselor,
• Documented reading level,
• Complete Lee College assessment/placement tests and complete developmental courses, if required,
• Demonstrate competencies in Allied Health Math 0110 or “C” or better on advance placement test for MATH 0110.
• Mandatory information session with VN faculty member.

Bonus Points

• BIOL 2404-Human Body-no older than 2 years, “B” or better is required. (If this course is taken prior to entering the VN program, the student will not have to take BIOL 2404, which is required during the fall semester of the VN program.)
  OR
  BIOL 2401 AND BIOL 2402-no older than 2 years, “B” or better is required. (If these courses are taken prior to entering the VN program, the student will not have to take BIOL 2404, which is required during the fall semester of the VN program.
• HITT 1305-Medical Terminology. (If this course is taken prior to entering the VN program, the student will not have to take HITT 1305 which is required during the fall semester of the VN program.
• COSS 300-Study Skills
• ITSC 1309-Integrated Software Applications I
• GPA (based on BIOL 2404 or BIOL 2401 and 2402, MATH 110 and HITT 1305) ? (4.0 = 1 point; 3.9-3.0 = .75 point; 2.9-2.0 = .50 point.)

Applications are taken year round; students are encouraged to apply by February 1 for fall entry. Obtain a VN admission packet in the Allied Health office or the Counseling Center. No points are calculated for admission after May 30th. Class size is limited and there are more applicants than slots Completion of recommended bonus courses does not guarantee admission to program.

After successful completion of this program, the graduate is eligible to take the National Council Licensure Examination for Practical Nurses. Upon passing the examination, the graduate is licensed as a vocational nurse.

The Board of Nurse Examiners has identified certain circumstances that may render a potential candidate ineligible for licensure as a registered nurse in the State of Texas. The Board provides individuals the opportunity to petition the Board for a Declaratory Order as to their eligibility in accordance with Article 301.257 of the Nursing Practice Act.

If you are required to answer “YES” to any of the following questions, please detach the request for a Declaratory Order Petition from the Board of Nurse Examiners, P.O. Box 430, Austin, Texas 78767-0430, and return it to the Board’s office. Processing your Petition may take 6 to 12 months, or longer, after you provide all required documentation and depending on your circumstance. Once all requested documents have been received, you will be notified that the Petition has been transferred to the Enforcement Department for review.

1. Have you been convicted, adjudged guilty by a court, plead guilty, not contest or nolo contendere to any crime in any state, territory or country, whether or not a sentence was imposed, including any pending criminal charges or unresolved arrest (excluding minor traffic violations)? This includes expunged offenses and deferred adjudications with or without prejudice of guilt. Please note that DUIs, DWIs, PIs must be reported and are not considered minor traffic violations. (One time minor in possession [MIP] or minor in consumption [MIP] do not need to be disclosed, therefore, you may answer “No.” If you have two or more MIPs or MICs, you must answer “Yes.”)
2. Do you have any criminal charges pending, including unresolved arrests?
3. Has any licensing authority refused to issue you a license or ever revoked, annulled, cancelled, accepted surrender of, suspended, placed on probation, refused to renew a professional license, certificate or multi-state privilege held by you now or previously, or ever fined, censured, reprimanded or otherwise disciplined you?
4. Within the past five (5) years have you been addicted to and/or treated for the use of alcohol or any other drug?*
5. Within the past five (5) years have you been diagnosed with, treated, or hospitalized for schizophrenia and/or psychotic bipolar disorder, paranoid personality disorder, antisocial personality disorder, or borderline personality disorder?
